Focus on Dementia

A partnership programme between the Joint Improvement Team, Alzheimer Scotland, the Scottish Government Mental Health Division and the Quality and Efficiency Support Team (QuEST) to: support the delivery of the Post-Diagnostic HEAT target across all Health and Social Care Partnerships; test the Alzheimer Scotland '8 Pillars' model for community based support in five areas in Scotland; support partnerships to use data to drive improvement; and support improvement in the care of individuals with dementia in acute hospitals.
